Data Privacy Notice

1. Your personal data – what is it?

Personal data relates to a living individual who can be identified from that data. The processing of that data is now governed by the General Data Protection Regulation (GDPR)

2. Who am I?

I am an ENT surgeon, who has been requested to provide a medical report on your head and neck problems by solicitors involved with your claim. To book your appointment for your consultation and to create the report, I am required to process your data.

3. How do I process your personal data?

I email your name, address, telephone number, date of birth and GP details to the hospital where you will be seen, to enable them to create your appointment. Your personal data will never be passed by me to others for marketing purposes, and will only ever be used in your legal case.

Confidential medical information contained in the report I write will be held on my computer securely, until I am informed the claim is closed.

The report will be sent to your solicitors or agents acting on behalf of your solicitors electronically in an encrypted format by secure email.

When I am informed the case is closed, I will delete the report from my computer and destroy any paper information on you in a secure way.

4. Sharing your personal data

Your data will be shared with your solicitors or their agents only.

Data processing is necessary to carry out my obligations to the solicitors dealing with your claim. Your confidential information will always be treated as strictly confidential.

I will not provide any of your confidential information to other parties, unless obliged to by law.

5. How long do I keep your data?

I will securely dispose of your data when I am informed that your case is closed.

6. Your rights and your personal data

You have the right to request a copy of your personal data held by me, and to request that I correct any inaccurate personal data.

You have the right to request your personal data is erased when it is no longer necessary for me to retain such data.

You have the right to withdraw your consent to the data processing at any time by contacting me using the contact details provided.
